Dear FSRA,
I have previously contacted FSRA regarding serious allegations of insurance-related fraud and financial misconduct connected to my case (R. v. Longo, Case 94545 / 21-845 and related proceedings). These allegations involve, at minimum, Sun Life Financial, estate payout diversion, and related financial instruments that form part of the documented pattern of abuse and exploitation spanning over two decades.
To date,
I have received no FSRA file number, no investigative update, no confirmation that any investigation has been opened, and no explanation for the silence.
This is not occurring in isolation. Multiple oversight bodies — LECA, CRCC, the Ontario Ombudsman, and the Attorney General's office — have all received formal demands for status updates today related to police and judicial misconduct in the same case. LECA File E-202606011107233651 is now subject to a 7-business-day deadline. The Windsor Police / CPIC evidence package naming Deputy Chief Karel DeGraaf (Badge #7815) is public at

FSRA's silence on the financial/insurance dimension is particularly notable because the insurance payout — a $707,000 death benefit paid within days — is one of the few independently verifiable financial events in this case. It does not require police testimony or judicial findings. It is a financial transaction with a paper trail.
What FSRA Has Regulatory Authority Over
• Insurance companies and agents operating in Ontario, including Sun Life Financial
• Conduct of insurance intermediaries and claims handling
• Financial misconduct, fraud, and misrepresentation in insurance matters
• Protection of consumers from unfair practices in the financial services sector
I therefore formally demand the following within seven (7) business days:
1. Confirmation of whether FSRA has opened or will open an investigation into the insurance and financial misconduct allegations referred to FSRA in prior communications.
2. Assignment of a formal FSRA file or reference number for this matter.
3. A written explanation for the absence of any substantive response or investigative action to date.
4. Confirmation of whether this matter has been coordinated with or referred to any other agency (including but not limited to the Attorney General's office, the Ontario Ombudsman, or law enforcement).
Should no meaningful response be received within the stated timeframe, this letter and FSRA's silence will be added to the public evidence record as further proof of coordinated institutional inaction across financial, police, and judicial oversight bodies.
